SVHOA Board Meeting

Ø Board members present: Mike Sobczak, Phil Raymond, Mike Recob, Joe Carroll, Matt Dana, Terry Emery

Ø Matt has 2004 YTD wages available for the officials that want them.

Ø Jim Irrer had an extra copy of the W-9 Form that is used to record official’s name and SSN. Mike Recob will make copies and bring them to the next meeting. Matt will collect the W-9 Forms from all officials and keep them on file.

Ø The SVHOA is recognized as a Non-Profit Organization and our tax ID # is 2557898

Ø The Board discussed the 2005 wages for Federal Income Tax purposes. Do we begin 1099 tax form for all officials starting Jan 05? YES The 1099 forms will be distributed to those officials that make more than $600 during the 2005 calendar year. It will be the responsibility of these officials that receive the 1099 tax forms to claim their income when they complete their 2005 Federal Income Tax paperwork.

Ø Special thanks to Joe Langschwager and Jim Irrer for their help with the Federal Tax scenarios, and setting the proper action plans that will make our SVHOA compliant to Federal Tax Laws.

Ø We will discuss pay raises of 20-30% for the upcoming seasons since the workload and responsibilities increase on our officials and Treasurer directly related to the compliance of Federal Tax Laws.

Ø The Missed Game Policy will be amended to include the MHSAA (high school) games. The recommendation is the person that misses their MHSAA assigned game will be removed from their next MHSAA assigned game. If a second game is missed during the season, the remainder of MHSAA assigned games will be removed.

Ø GSAHA Friendship Tournament for the Mites & Bantams starts Friday 14JA05. The Tournament will continue the following Friday 21JA05 with the Squirt & PeeWee divisions.

MEMBERSHIP MEETING (50 in attendance)

Ø Phil informed the group that the GSAHA Friendship Tournament begins Friday 14JA05 and runs thru Sunday for the Mite & Bantams. Next Friday 21JA05 thru Sunday will be the Squirt & PeeWee Friendship Tournament.

Ø GSAHA Friendship Tournament (cont.)

o Officials shall show up to their assigned games 40 min before game time.

o Look presentable / professional…NO Jeans

o If you wear a ball cap, wear it properly

o Joe will post Tournament Rules on the Ref Board

o No time outs

o Overtime will be played in Semi & Finals unless specified in rules posted or governed by Dave Westner.

o Game sheets (score sheets) will be signed by coaches (legibly) and contain their USA Hockey Coach Certification #.

o Officials will Print their names on the score sheet and write their # next to their names. This is a good practice to use for all your games. Make it a habit!

o Have fun!!!

Ø Next Meeting scheduled for 01-Feb-05. SVHOA Board Meeting at 6:30pm with the Membership Meeting scheduled immediately following (7pm).
